When planning a thorough program evaluation, you will want to go through a general planning process (such as is described in the websites and documents on the General Resources page). This planning process will lead you to develop a range of tools and instruments. However, sometimes you might just want to look at some simple forms to get ideas outside of a more formal process. On this page we'll collect some samples for you.

Checklist for planning an evaluation
= This checklist provides a general set of questions that are useful to consider before undertaking a program evaluation study. It is recommended for use by a small planning team. Basic Logic Model
= A generic logic model that is often used separates program activity into inputs, outputs, and outcomes as in this overview document: Logic Model.pdf. A blank doc version of the same template can be downloaded here: Blank Logic Model Flowchart.doc

Feedback Forms

= Two simple forms for responding to a workshop and summarizing those responses; useful, for example, for conference sessions or one-of's:
